The PDF application should only be used by an applicant who:

  • is applying for admission to a graduate program in an off-cycle semester,
  • is a graduate student at the University of Utah who did not maintained continuous registration and failed to file a formal “Leave of Absence” form with the University and, therefore, the applicant’s graduate program was discontinued, or
  • is in need of an ADA alternative format.
